Documents are mandatory for uploading reimbursement claim

Online Individual Reimbursement Claim Facility.


A facility has been created to enable an ECHS beneficiary to upload all individual reimbursement claim directly on the BPA portal. Instructions are as follows:-
  1. Enter the 12 digit card number as mentioned on beneficiary/ dependent 64 KB ECHS Card.
  2. Enter the mobile number which is registered with 64 KB ECHS Card.
  3. If the card number and mobile number are correct then an OTP will be sent on the registered mobile number.
  4. In a month only four IPD claims can be submitted online.
  5. In a month only four OPD claims can be submitted online.
  6. In a month only eight Pharmacy Bills can be submitted online.

The following documents are mandatory for uploading reimbursement claim:-

IPD.
  • ECHS Card Copy.
  • EIR in case of emergency.
  • Emergency Letter (By Hospital).
  • Bill Details.
  • Discharge Summary.
  • Medical Reports.
  • Copy of Cancelled Cheque.
OPD.
  • ECHS Card Copy.
  • Prescription Slip.
  • Bill Details.
  • Medical Reports.
  • Copy of Cancelled Cheque.
Pharmacy (NA Medicine Reimbursement).
  • ECHS Card Copy.
  • Prescription Slip.
  • Bill Details.
  • NA Certificate.
  • Copy of Cancelled Cheque.
